Insurance:
We accept Medicare, Workers' Compensation and most PPO plans. In addition to contracting with over 200 national insurance companies, each of our locations maintains contracts with local insurance groups.
As a courtesy to our patients, we will obtain prior authorization for your care and will bill your insurance company directly.
Prior to or at the time of your first visit we will be happy to discuss financial arrangements and determine if your insurance is accepted.
We are pleased to offer our forms online to ensure that your visits with us are as time efficient as possible. You may fill them out at your convenience and bring them in at the time of your first appointment.
